St. Elizabeth’s Medical Center’s Palliative Care Program helps patients understand the nature of their illness and make timely, informed decisions about their care, together with their families. Seriously ill patients benefit from palliative care at any stage of their illness.
Appropriate consults include: end of life care; goals of care discussion; advance directives; and pain and other uncontrolled symptom management, such as dyspnea, nausea, constipation, and delirium. The palliative consultant works in collaboration with the primary team of physicians, case management, home care, and hospice to ensure optimal transition of care for the patient.
