In the event that you need skilled nursing or rehabilitation services in your home, Steward® Geriatrics will coordinate with Steward Home Care to ensure continuity in your treatment. Our physicians and nurse practitioners will make home visits as needed.

St. Elizabeth’s Medical Center’s Palliative Care Program helps patients understand the nature of their illness and make timely, informed decisions about their care, together with their families. Seriously ill patients benefit from palliative care at any stage of their illness.
Dr. Asif Merchant, chief of Geriatric Medicine and director of the Palliative Care Program, is available for consultation. Appropriate consults include: end of life care, goals of care discussion, advance directives, and pain and other uncontrolled symptom management, such as dyspnea, nausea, constipation, and delirium. Dr. Merchant is also a Hospice Medical Director. The palliative consultant works in collaboration with the primary team of physicians, case management, home care, and hospice to ensure optimal transition of care for the patient.
