An Integrated Model of Care
At Steward Geriatrics, we understand that the health needs and concerns of older adults can be complex. Our specialists in geriatrics, joint replacement, diabetes, heart disease, rheumatology, neurology rehabilitation and memory disorders take a multidisciplinary, collaborative approach in providing comprehensive care. Our physicians and nurse practitioners are board certified in geriatrics offering individualized care to meet your primary care needs.
Someone from our team of doctors and nurse practitioners is always available by phone 24 hours a day and 7 days a week.
We coordinate your care in a variety of settings — in our outpatient clinic, your home, in the hospital if you are an inpatient, at a rehabilitation facility, after a surgical procedure or in another long-term setting. If you are receiving care outside your home, our physicians and nurse practitioners will manage your care to ensure and facilitate a smooth discharge home. Should you need alternative living to an assisted living community or long-term care setting, our team will work closely with you and your family to make this transition a positive experience. To ensure your comprehensive care, we collaborate with many other health care providers such as local visiting nurse associations, Steward Home Care, elder care services, senior centers and hospice and palliative care organizations. We can provide you with referrals to St. Elizabeth’s specialists, and if you require hospitalization, we will provide and/or coordinate your inpatient care. Our goal is to provide you with a feeling of security about all of your health care needs.
