Treatment Options by Stage of Bladder Cancer

The stage of a cancer tells the doctor the extent or severity of a person’s cancer and whether it has spread. The tests and scans you have when diagnosing your cancer give some information about the stage. Knowing the stage of the disease is important because it helps the doctor plan the appropriate treatment.

Stage I Bladder Cancer

Treatment of stage I bladder cancer may include the following:

  • Removal of just the cancer by performing a surgery called a transurethral resection of a bladder tumor (TURBT) with or without chemotherapy inserted directly into the bladder
  • Segmental or radical cystectomy (partial or full removal of the bladder)
  • A clinical trial of chemoprevention therapy given after treatment to stop cancer from recurring
  • A clinical trial of intravesical therapy (treatment that is put directly into the bladder through a catheter) 

Stage II Bladder Cancer

Treatment of stage II bladder cancer may include the following:

  • Radical cystectomy
  • Combination chemotherapy followed by radical cystectomy
  • External radiation therapy combined with chemotherapy
  • Transurethral resection with fulguration (destroying cancer cells with a high-frequency current)
  • Segmental cystectomy

Stage III Bladder Cancer

Treatment of stage III bladder cancer may include the following:

  • Radical cystectomy with or without chemotherapy before or after surgery
  • External radiation therapy combined with chemotherapy
  • External radiation therapy

Stage IV Bladder Cancer

Treatment of stage IV bladder cancer may include the following:

  • Radical cystectomy
  • External radiation therapy
  • Chemotherapy alone or before and after local treatment (surgery or radiation therapy)
  • Urinary diversion as palliative therapy to relieve symptoms and improve quality of life
  • Participation in clinical trial with novel therapy
